COBRA Benefit Renewal


This section walks you through completing your COBRA renewal with Alerus. It applies to all COBRA eligible benefit plans and must be submitted at least 30 days before your plan and rate renewal date.

Please submit all finalized renewal information in one email to cobra@alerus.com. Additional fees may apply if incorrect information is submitted and corrections are needed.

COBRA Renewal Documents

Fill out all sections of the COBRA renewal documents that apply to you. Detailed instructions for completing and submitting your COBRA renewal are provided on the first tab of the spreadsheet.

Plan Information Audit

  • Before submitting a renewal, you must review the current plan details in Alerus to determine if any updates are needed. After the renewal is completed, you should confirm that all pending plan updates have been processed.
  • To view current plan records in Alerus:
    • Log in to your employer Alerus COBRA account.
    • Select Qualified Beneficiary>Plans.
    • Click the eye icon under each plan to view specific details and rates for each plan.

Responsibility for Accuracy

  • Under the terms of our contract, the Employer is solely responsible for ensuring that all renewal and premium information submitted to Alerus—whether by the Employer directly or through a benefits broker or consultant—is complete and accurate. Alerus is not responsible for verifying the accuracy of this information and will rely on it as provided. Alerus assumes no liability if the renewal or premium information received is inaccurate.
  • If renewal information is submitted after the renewal date, premium changes for continuation members will take effect on the first day of the month following the date the information is received. Premiums cannot be retroactively adjusted for late submissions. Alerus is not responsible for any premium discrepancies that result from delayed renewal information.
